Magnolia Care Solutions provides care management and guardianship services to benefit individuals who are in need of physical, mental, social and/or emotional support. All services are tailored to impact and improve our client's life. We work with family members, guardians, trust officers, attorneys, judges, and other interested parties to facilitate care and advocate for our clients.

Developing thorough Plans of Care and Independent Care Management Reports based on comprehensive needs assessments.

Assisting trustees and Guardians of the Estate with annual budgets.

Researching and arranging for appropriate medical care, home health services, caregivers, therapies, cleaning services, and household repair needs.

Identifying adaptive equipment needs, home modifications, and transportation services.

Arranging recreational and social activities

Coordinating educational supports

Resolving insurance needs and benefit eligibility

Making arrangements for hospice and end of life planning

Attending IEP's and medical appointment for minors with disabilities to provide advocacy and family support.

Performing crisis intervention during medical and psychiatric emergencies.

Testifying in probate court as requested by families and attorneys for guardianship trials

Providing support to individuals without local family or guardians.

Performing wellness checks to monitor care provided to individuals residing independently or in medical facilities.

Acting as an intermediary between families and parties in dispute over client care needs.

Providing supervised visitation.

Travel companionship.
